New Emergency Contact Sheet

High School and Middle School Parents should have received a new personalized Emergency Contact Sheet in the mail. The Elementary Schools will be mailing their personalized Emergency Contact Sheets this week. Parents should receive a separate contact sheet for each student they have in the Jericho Schools. If you do not receive a sheet, please contact your child's school.

The personalized form contains the contact and demographic information that is in our system. We are asking parents to review and correct the information on the form. While reviewing it, parents will notice that, for the first time, we are asking for parent e-mail addresses. Beginning next year, we will be e-mailing as much school information home as is possible. Paperless communication will result faster dissemination of information while resulting in significant savings in paper, xeroxing, envelopes and postage.

We will also be creating a password-protected, confidential Web page for your child. That Web page will provide links to:
• Contact forms (all schools)
• Standardized Test Reports (when available)
• Report cards and progress reports (Middle and High Schools)
• Other documents

Parents who include e-mail addresses on their forms will be receiving a username and password that will enable them to access their child's folder. Middle and High School parents who return their contact sheets before May 15 will receive a username and password by the middle of June. Final quarter report cards will be available on-line. All others will receive their usernames and passwords during the summer.
